Department of Surgery


 

1.   Introduction:

Since the inception of the hospital, the department of Surgery has been providing surgical services in peacetime, conflict situations, and disasters. Surgery department is one of the major service providers in Shree Birendra Hospital. Department provides comprehensive surgical services in general and surgical subspecialties including gastrointestinal- hepatobiliary pancreatic (GI-HBP), urology, neurosurgery, cardiovascular-thoracic surgery, surgical oncology, minimally invasive surgery (MIS), plastic surgery, and trauma surgery. Shree Birendra Hospital was the pioneer in daycare services like ESWL (extracorporeal shock wave lithotripsy) and other minimally invasive surgery and was the first hospital to introduce ESWL in Nepal.

The Department is a major provider of acute surgical care and tertiary surgical services in Nepal. It provides specialist care for patients, teaching for graduate and postgraduate, training for doctors and other healthcare professionals also researches to bring better care to our patients. Currently, department of surgery consists of two general surgery units, the urology unit, GI-HBP unit, neurosurgery unit, and cardiothoracic and vascular unit. Besides existing units and services the department is planning to provide services in another field including liver and kidney transplants. We have achieved status as a tertiary-level referral center in Nepal.

2.     Services:

The department of surgery provides the following services:

a)    Outpatient services- 6 days a week (Sunday - Friday)

4.     Subspeciality services:

·         Gastrointestinal-Hepatobiliarypancreatic (GI-HBP): The unit of Gastroenterology & hepatology is adept at managing a wide spectrum of gastroenterology and hepatology diseases of varying severity. The unit has highly skilled and dedicated specialists in various procedures, including advanced diagnostic/ therapeutic procedures. All procedures and investigations are performed in an environment with adequate facilities.

·         Urology unit: The unit is one of the major service providers of the department of surgery in Shree Birendra Hospital. The urology unit has been dealing with the basic urology services and now it has progressively been expanded to advance urology and endo-urological services to MIS (including ESWL, PCNL, RIRS, and HOLEP). The urology unit is established to provide a one-stop evaluation and treatment center for all types of urological conditions. The specialist unit and evaluation rooms in the department are fully equipped and present of well qualified specialists to treat patients in the areas of many urological problems including uro-oncology and female urology.

·         Neurosurgery unit: The unit comprises a team of highly trained specialists, nurse clinicians, and physical and occupational therapists who are all equipped to provide patients with care in the field of neurosurgery. The unit has highly trained specialists who offer extensive expertise in areas such as the treatment of brain tumors, neurovascular disorders, spine disorders, and head injury. The neurosurgery unit has well set up separate ICU facility.

·         Cardiothoracic and Vascular unit: The unit carries out all basic and minimally invasive general thoracic surgery for the diagnosis and treatment of a spectrum of chest, lung and vascular diseases. The unit has dedicated specialists who also provide emergency trauma care to patients with chest injuries from local and regional center of Nepal.

9.     United Nation (UN) participation of surgeon:

Following Nepal Army's active participation in UN Peacekeeping Operations, Nepal has remained as one of the major troops contributing nations to the UN over the last few decades and has managed to portray the image of a professional, disciplined, dedicated, dependable and probably that of one of the best soldiers in the world.

In the more recent history of its participation in the UN peace keeping force, Nepal army has deployed two level one plus hospitals in Syria and Libya following the need of modernization and diversification of its troop's contribution. The first Level one plus hospital was established in UNSMIL, Libya in October 2017, which consisted of an AMET module and a forward surgical module in addition to a regular Level one medical facility.  This was followed by establishment of a UNOE Level one plus hospital in Syria in July 2019. Since their inception, both the hospitals are functioning as the forward surgical module with critical care facilities providing these advanced specialist care to the UN staffs. Deployment of these hospitals has provided Nepal army with an opportunity to show its medical capabilities, diversify its participation in UN and further strengthen the bilateral relationship between Nepal with the United Nations

 

10.Summary of Department activities:

·         Timing: 9 am to 4 pm (Depend on work load).

·         Morning meet (audit): Sharp 9.00 to 9.30 am morning (Opd class room).

·         Rounds: Round start sharp 9.30 am with evening round at 3.00 pm.

·         Grand Rounds: Every Friday 9 am to 10 am.

·         24 hours duty time: 9 am to next day 9 am.

·         Reporting time by phone: Every day 8 pm to 9 pm by on call resident to respective unit in charge.

·         OPD time and day: As per unit schedule. Priority given to prior appointment.

·         OT day: As per unit schedule.

·         Lunch Break: 1.00 pm to 2.00 pm.

·         Leave: As per government rules and regulations (but tentative leave plan should be notify during beginning of every New Year).

·         Academic activities: As per TUTH curriculum and NAIHS guideline.
